The dental rates are determined by the Dutch Healthcare Authority (NZa). Health insurers refer to the document with the dental rates as the dental care policy rule of the Dutch Healthcare Authority.

You are responsible for the appointment yourself; if you are unable to comply with this, please call or leave a message on the answering machine at least 24 hours in advance. Otherwise we are forced to send an invoice and charge the scheduled time.
